Below, you will find the print version of both the maintenance request form and various other forms that may assist you in adding, removing or switching roommates.
• Maintenance Request Form Submit a maintenance request via fax or email.
• Automatic Draft Authorization Never be late on a payment again! Give authorization for Barker to draft your rent each month on a predetermined date.
• Forwarding Address Form Make sure we have a way to send your Security Deposit back to you. Please provide us your forwarding address upon moveout.
• Move Out Instructions Make certain you receive your complete Security Deposit. Look over these requirements to ensure your property is in tip top condition upon move out.

• Vacate Notice It's in your lease! We need to know when you are moving out. Check your lease or contact your Property Manager to verify what your notice timeframe will be.

• Application Form - Co-signer (Traditional Only) Use a co-signer to meet the minimum income requirements on a property in the traditional portfolio.
